The Home & Community Care (HACC) program provides a range of basic services designed to support frail older people, people with disabilities, and carers to stay active, independent and living at home for as long as possible. Supporting people to stay active, involved and doing as much for themselves as possible is proven to help people stay more independent, with a greater sense of control and wellbeing.

 

Help Around the Home

In-home services to help maintain a safe and hygienic living environment and may include support with domestic duties and property maintenance.

Self-Care

Personal Care support to manage tasks such as shower and dressing assistance.

Meals

A range of support options including shopping assistance, assistance to prepare meals, or the provision of delivered meals in the home.

Shopping

A range of supported shopping options including group shopping, trips, escorted shopping and unescorted shopping assistance.

Out and About

Connections to interest groups or social opportunities including: planned activity groups, senior centres, friendly visitors and transport to medical related appointments.

Respite

Relief for the primary carer of an older person or people with a disability by providing a planned break from caring responsibilities.

Accessing Services

Information about how to access services provided by the Council and other community organisations within the area.

Volunteering Opportunities in Aged & Disability Services

There are a number of volunteering opportunities available within the Aged and Disability Services, working with clients who are frail aged or have a disability.

What is HACC?

HACC stands for Home and Community Care and is funded by the Commonwealth, State and Local Governments.

Veterans Home Care

VHC is a Department of Veteran Affairs (DVA) program that supports veterans and war widows with low care needs to remain in their own homes.
